Checklist – Visa, US B1-B2

US Tourist Visa Checklist for Family:

1. Passport (original) valid for at least 6 months from the date of travel to US before applying for a visa, with two blank sheets back-to-back blank pages (Passports valid for more than 10 years will not be accepted) along with old passports for all Applicants.
2. Online Completed DS160 application form Confirmation page along with Appointment scheduled receipts and visa fees paid receipts copies for all the Applicants.
3. Original personal covering letter in personal letter head OR plain white sheet stating the purpose of travel, duration of stay, travel dates, All Applicants Passport BIO Page details, mentioning expenses borne by whom etc with request to issue the necessary visa. (Address the letter to: The Visa Officer, US Consulate General, Chennai, India, OR Respective Consulate where the Personal Interview Appointment fixed Consulate in India. (PFA Format and it is just an Example, kindly update the correct details).
4. Hotel Confirmation copies, Confirmed Tickets, and other online booked entry tickets OR USA Sponsor documentation like Sponsor BIO page of the passport copy, Sponsor last 6 months pay slips and bank statements (updated) clear colour scan copies, Sponsor property tax bill copy, Sponsor Employer Bonafide letter clear colour scan copy, Sponsorship declaration AFFIDAVIT OF SUPPORT which attested by US government, Sponsor Formal Invite letter address to Main Applicant by mentioned all family members name and passport numbers.
5. Both Husband and Spouse original Bank statement where the salary is credited for last six months updated, with details of the bank name/address, Applicant carrying Bank seal & Manager sign on each page from the bank The name and account number should be visible on the bank statement with sufficient Balance. (Updated statement can be taken close to visa interview date) (mandatory).
6. Both Husband and Spouse Last six months’ pay slips with HR sign and Company seal along with Appointment order copy / Recent Promotion Order.
7. Both Husband and Spouse Original BONAFIDE CERTICATE from Company by mentioning the Full Name, Designation, Date of Joining, Current Residence address, Contacts Mail & Phone, sanctioned leave approval for the travel dates. This should be in Company letter head which duly signed by Authorized Signatory with Company seal and Signing Authority contact details.
8. Both Husband and Spouse Personal Income tax returns Original & copies of last three years.
9. Previous work Experience certificates along with last 3 Months payslips and all higher degree educational certificates (Both Original and Clear colour Copies) Optional US Consulate May Request at the time of Personal Interview. (For both)
10. Kids Birth Certificate copies along with Student ID Card and Original School Bonafide Certificates Includes leave Approvals for the travel dates.
11. Marriage Certificate copy.
12. Aadhaar Card copy, Vaccination certificate copy, Pan Card copy, and other Govt Issued ID copies and residential proofs of all Applicants.
13. Previous Passport originals and copies along with Previous Visa pages of all Applicants.
14. Recommended to carry other financial documents like Mutual Funds, Share certificate copies, Fixed deposits, Other Savings account holding Bank statements, If any to prove that all the Applicants are Financially sound in India to meet any expenses during travel USA, and this may support confirming that will be back to India.

Note: US Consulate may request additional documents after submission to check for more verification and take decision on visa Issuance of visa would be solely under the discretion of Visa officer / The above checklist is the standard checklist. However, the US Consulate reserves the right to ask for additional documents which are not mentioned above.
